When Pregnancy Needs Closer Watching

Not every pregnancy runs a straight course. Some need extra scans, tighter sugar control, more frequent blood pressure checks, or simply a doctor who is reachable when something feels off at 9 PM. At Borivali Women & Child Clinic, Vazira Naka, we manage high-risk pregnancies with a schedule that is intensified where it matters and left alone where it does not – because over-testing creates anxiety, and under-testing creates emergencies.

Conditions We Manage

Gestational diabetes (GDM). Screening at 24-28 weeks, earlier if you have PCOS, a family history or a previous large baby. Management starts with a Borivali-realistic diet plan – one that accounts for chapati, poha, upma and evening chai – before insulin is ever considered.

Pregnancy-induced hypertension and pre-eclampsia. Blood pressure at every visit, home BP monitoring taught in the clinic, protein-in-urine checks, low-dose aspirin prophylaxis where indicated, and clear warning signs for headache, blurred vision or swelling.

Thyroid disorders. Hypothyroidism is very common in Mumbai women. We recheck TSH every six to eight weeks and adjust the dose rather than continuing the pre-pregnancy tablet unchanged.

Twin and triplet pregnancy. More frequent growth scans, cervical length monitoring, earlier planning for delivery and a neonatologist alerted well in advance.

Previous caesarean, previous preterm birth or previous loss. Scar thickness assessment, cervical length surveillance, progesterone support where appropriate, and a delivery plan agreed with you months ahead.

Anaemia. Very common locally. We treat with oral iron, and with iron sucrose infusions when haemoglobin is low or the third trimester is approaching fast.

IVF and IUI pregnancies. Extra luteal support, closer early scanning and careful counselling, since these pregnancies carry a higher chance of twins and of maternal anxiety.

Rh-negative mothers, placenta previa, oligohydramnios, growth restriction, cholestasis of pregnancy and advanced maternal age are all managed with individualised protocols.

What Closer Monitoring Looks Like

You can expect visits every two weeks from the second trimester and weekly from 32 weeks, additional growth and Doppler scans, home charting of sugars or blood pressure, a written escalation plan telling you exactly when to come in immediately, and an admission plan made in advance with the hospital rather than in the middle of a crisis.

Why This Care Works Better Locally

Living in Borivali means long commutes, crowded trains and unpredictable evenings. A high-risk pregnancy plan made for a patient in a different city does not survive contact with the Western Express Highway at 7 PM. We build your schedule around your actual travel and work, keep evening slots free until 10 PM, and keep the phone lines open so a worrying symptom gets a real answer instead of a wait.

Frequently Asked Questions

What makes a pregnancy high-risk?

Age above 35 or below 18, diabetes, high blood pressure, thyroid disease, obesity, twins, a previous caesarean or preterm birth, recurrent miscarriage, IVF conception, or a medical condition such as heart, kidney or autoimmune disease.

Does high-risk always mean caesarean?

No. Many high-risk pregnancies end in a normal delivery. The label means closer monitoring, not a fixed outcome, and the delivery route is decided near term based on how mother and baby are doing.

I have gestational diabetes. Will my baby get diabetes?

Your baby will not be born diabetic, but poorly controlled sugars can cause a large baby, low sugar after birth and jaundice. With good control, outcomes are usually excellent. Your own risk of type 2 diabetes later does rise, so postnatal screening matters.

How often will I need scans?

Typically every three to four weeks in the third trimester instead of the usual two scans, with Doppler studies if growth is a concern. We order what is needed and skip what is not.

When should I come in immediately?

Bleeding, leaking fluid, severe headache with blurred vision, upper abdominal pain, fever, reduced fetal movements, or contractions before 37 weeks. We give every high-risk patient this list in writing.
